• Membership Application (Application will be electronically sent to MHWC when you click “Submit” below)
• Membership Agreement (Form located in “Forms Library”)
• Inventory Enrollment Form (Editable form located in “Forms Library”)
• Incentive Program Agreement (Contact your Account Executive for applicability)
• Non-refundable Application Fee of $295. (Mail to MHWC or call to pay by credit card.)
• Underwriting will notify you if any additional information may be needed based on experience, program selection, type of homes to be enrolled, etc.
• Copy of Insurance Certificate from CGL Carrier (Upload Mail/E-Mail to MHWC)

26. INVESTIGATION: The Applicant firm hereby authorizes MHWC to conduct such investigation of the Applicant’s activities and make such inquiries and obtain credit reports as may be necessary for its determination of Applicant’s financial and technical ability to meet its obligations to purchasers. Applicant hereby directs all credit reporting agencies to make available to MHWC any information in the possession of such agencies.
I hereby attest that all information provided to MHWC in conjunction with this Application for Membership including all attachments is true and correct including the attached financial statement. I understand that I will be held personally responsible for any loss incurred by MHWC as a result of any and all falsified information and that provision of falsified information is a breach of the Membership Agreement and grounds for immediate cancellation of membership in the MHWC program.
